The Community Health Coordinator IV for the UofL Kentucky Care Coordinator Program (KCCP) supports HIV-positive individuals eligible for Ryan White Part B services in the KIPDA region. This role helps clients overcome barriers to care and supports long-term retention through care coordination, transportation assistance, and referrals to medical and support services.
Working with providers, case managers, and community partners across Kentucky and Southern Indiana, the coordinator ensures timely linkage to care, maintains client eligibility, and supports compliance with Ryan White program requirements. The position plays a key role in promoting positive health outcomes for people living with HIV.
Essential Duties and Responsibilities
Serve as the assigned Care Coordinator for low-acuity clients eligible for Ryan White Part B services in the KIPDA Region, ensuring timely enrollment and eligibility updates every six months.
Conduct acuity assessments and develop/update Individualized Care Plans (ICPs) based on client needs, coordinating referrals to appropriate support services.
Assist with insurance education and enrollment support, helping clients navigate kynect or federal platforms and referring to partner brokerage agencies as needed.
Provide health education and advocacy to newly diagnosed or re-engaging clients, linking them to care and referring HIV-negative individuals to prevention services.
Accompany clients to medical or outreach visits as appropriate to support engagement and reduce barriers to care.
Coordinate with KCCP and 550 Clinic Medical Case Managers (MCMs), providers, and community partners to ensure seamless care transitions and warm handoffs.
Maintain accurate documentation in Provide Enterprise, EMR, and other systems to support quality improvement and Ryan White program reporting.
Monitor and follow up on missed appointments, ensuring clients stay engaged and connected to care.
Build and maintain strong relationships with community partners to expand referral resources and improve client access to support services.
Participate in required trainings, CQI meetings, outreach activities, and complete administrative duties, including monthly reporting and document management.
Perform other duties as assigned.
Preferred Qualifications
Minimum of 3 years of experience working with diverse populations, including individuals facing significant socio-economic barriers to health, such as homelessness.
Experience working with People Living with HIV/AIDS (PLWHA) and familiarity with the Ryan White HIV/AIDS Program (RWHAP) is highly desirable.
Demonstrated experience in clinical problem-solving, critical thinking, and delivering patient-centered care.
Proven teamwork, strong organizational, and time management skills in fast-paced clinical or community health settings.
Proficient in Microsoft Office, particularly MS Word; experience with electronic medical records (EMR) and case management systems preferred.
Compensation will be commensurate to candidate experience.
